Facilities Use Request

I have read and understand the Trinity Anglican Seminary Facility Use Policy. By submitting this application, I agree that any use of Trinity facilities will conform to the Facility Use Policy. I acknowledge that the Trinity campus is an alcohol, tobacco, weapons, and illicit drug free campus. By signing this application, I agree to insure that no one who is part of my ministry, organization, or family brings any of these prohibited items on the Trinity campus. I have read and understand both the Trinity Covenant and the Trinity Statement of Faith. By signing this application, I agree to insure that no one who is part of my ministry, organization, or family engages in any conduct that is not in conformance with either the Trinity Covenant or the Trinity Statement of Faith.

The undersigned, their successors or assigns, hereby agrees to save and hold harmless Trinity Anglican Seminary and any of its Board of Trustees, Faculty, Staff and employees from, cost, injury and damage incurred by the use of any Trinity Anglican Seminary facilities or property, and from any other injury or damage to any person or property whatsoever, any of which is caused by an activity, condition or event arising out of the use the facilities or property.
